Watch whether they ask you anything back. 'I haven't used that, let me look into it' is a completely different energy than a flat no with zero follow up questions. First one is engagement, second one is just a wall.

For anyone reading this later, the practical order is compounding pharmacy first if she can get a script, telehealth as the backup path if local access is thin, and only look at online vendors as a last resort with independent COAs on every batch. Post surgical is not the situation to cut corners on sourcing, the margin for error is just too small right after an operation.

Telehealth has actually filled some of the gap for people who can't get a local compounding pharmacy visit. A licensed provider does an actual consult, writes the script, and it still routes through a real 503A pharmacy on the back end. Not the same risk profile as a random research chem site at all.

Compounding pharmacy is still the gold standard if she can get there. 503A or 503B pharmacies are inspected and tested, and you need an actual prescription from a licensed clinician, not just a telehealth click through form with no real visit behind it. The issue is access has been spotty since the FDA put a bunch of peptides on a restricted category back in 2023.

Neither one is FDA approved for any use, full stop. The legit pathway is a licensed clinician working with a compounding pharmacy operating inside whatever the current framework allows. That's a narrower door than people think right now but it's the only one that's actually legal and quality controlled.

Agreed it's not a protocol, it's anecdote, but anecdote from someone actually monitoring labs and recovery markers is still more useful than guessing blind off a YouTube video. That's the whole point of having a clinician in the loop instead of self dosing off forum numbers.

Had a conversation with one of our surgical liaisons last week that I haven't been able to shake. Patient six days out from an ACL graft wants to start BPC-157 for the tendon healing piece but she's still on a DOAC for a prior clotting history. I know BPC-157 and TB-500 both push angiogenesis, and I can't find anything on whether that creates any real interaction risk with anticoagulants. Anesthesia itself I'm less worried about since she's a week out, but has anyone here actually seen a post-op patient run into trouble combining peptides with blood thinners, NSAIDs, or opioids? Not looking for guesses, looking for what people have actually observed.

At my practice we require disclosure before combining anything peptide related with PT, not to gatekeep but because the blood thinner interaction gap is real, there's essentially zero published data on how BPC-157 or TB-500 behave alongside anticoagulants or antibiotics. Absence of reported problems isn't the same thing as proven safe, it just means nobody's looked. I'd rather have the surgeon and whoever's running the peptide protocol both know what the other has the patient on.

The honest answer is that cycle length should track the tissue healing timeline, not a forum number. Graft revascularization for something like a patellar tendon autograft takes months, so a two week run isn't doing much for that structure. What actually matters is having someone monitoring the recovery alongside the protocol, checking in, adjusting, pulling labs if something looks off, not just handing over a script and disappearing.
