Different peptides for graft healing vs tendon vs incision scarring?

Got a client six weeks out from ACL reconstruction with a hamstring autograft, and another one who just had a rotator cuff repair. Both are asking me about peptides because they've seen stuff online. I keep seeing BPC-157 and TB-500 thrown around like they're interchangeable but I'm guessing graft healing, tendon repair, and incision scarring aren't the same problem. For those of you who've actually coached people through this, is there a real difference in what you'd reach for depending on what got cut or reattached, or does everyone just run the same stack regardless?

For graft healing specifically I've seen guys lean on BPC-157 because the angiogenesis piece matters when you're trying to get a tendon graft to revascularize inside the joint. TB-500 gets added more for the early inflammatory window and general tissue migration. With the hockey guys I consult for post ACL, it's almost always both together, not one or the other.

All of that mechanism talk is solid but it's coming from rat transection models, not graft revascularization in human knees. Anyone have actual graft outcome data, even anecdotal timelines, or is this just extrapolation stacked on extrapolation?

It's anecdotal but it's not nothing. I've had post-op athletes on BPC-157 hit ROM milestones weeks ahead of their peers.

Lost my career to a knee. Now I fix them.

If the concern is incision healing and scar quality, topical GHK-Cu is the one with actual trial data behind it, the diabetic wound closure work is real. For deep graft or ligament tissue it's not your primary tool though. I'd also want vascular status checked before anyone gets aggressive with loading regardless of what peptide they're running.

Worth separating out what each one is actually doing. BPC-157 is proposed to upregulate VEGF and EGF, which drives angiogenesis at the repair site, that's the theoretical basis for use in tendon, ligament, and muscle. TB-500 is a thymosin beta-4 fragment and its story is more about cell migration and dialing down the early inflammatory response, which is why some people layer it in during that first couple weeks post-op. None of this has been run in a controlled trial on actual post-surgical patients, so you're relying on mechanistic reasoning plus anecdotal logs. That's not nothing, but be clear eyed that you're filling gaps with extrapolation, not outcome data from people who had your exact procedure.

Update, client's PT actually green lit trying BPC-157 alongside the rehab program, starting slow. Will report back.

I've seen too many people think the peptide replaces the work. No injection fixes a joint that isn't being loaded properly.

Nobody said it replaces loading. It's a tool on top of the program, not instead of it.
